We create individually tailored support packages for adults with complex care needs, including learning disabilities, mental health conditions, physical disabilities, and substance abuse issues.

Crisis Management

For people with complex care needs, a crisis can take many forms. It may be that someone with a mental health condition is experiencing an especially difficult episode. A person with substance abuse issues could be going through a period of relapse. Whatever the cause, a crisis requires the right support, fast.

Our crisis team responds to emerging crises promptly and effectively for service users across Plymouth and beyond. With access to experts from a range of disciplines, we deliver the urgent care that's needed—whether it's social or psychological support or practical help to meet the service user's day-to-day needs. We can help minimise the harm that a crisis can cause and be a source of comfort, strength, and stability during the lowest of points.

Supported Living

At Clear Thinking Care, we understand that every service user's situation is different. Some of our clients with complex care needs have learning difficulties and mental health conditions. Others live with a physical disability or sensory impairment. Whatever the situation might be, our team of caring experts is here to help make living at home as comfortable as possible.

We address a wide spectrum of day-to-day living needs. That can mean assisting with personal care and domestic chores, such as preparing meals and cleaning. It can involve helping with paying bills and taking care of money matters. Whatever the individual needs, it's our mission to deliver it—so they can experience the dignity and independence that come with living successfully at home.

Enabling

Enabling means different things for different people. At Clear Thinking Care, our care workers are adept at stepping in to give support across a wide range of areas, according to the individual's needs. Out-reach & In-reach

Through our Outreach service, we give people with mental or physical disabilities the knowledge and confidence they need to live well at home. Our team assists across a spectrum of life skills to bring meaningful and sustainable change.
