Requesting support and self-assessments
Self-assessments help you identify the areas where you might be eligible for social care and support.
At the end of the form, you have the option of making a referral and requesting a formal assessment.
You can complete the form for yourself or on behalf of someone else.
We will contact you once you have completed the form to explain what happens next. We will always try to do this within an appropriate and reasonable timeframe, based on the urgency of your needs and any changes in those needs.
Remember, if you are struggling, you are not alone. There is no shame in asking for help. Everyone needs a little bit of extra support at some point in their lives.
