As we age or live with a disability, there may come a time when we require additional support to manage our daily activities and maintain our independence. If you have any care or support needs, you can ask for a needs assessment.

If you need to pay for your care and support, we'll tell you how much we'll pay towards your care, how much you need to pay and how you pay it. You have the option to pay by direct debit, online, using the telephone, or in person.

Once we have received your request for an assessment it is passed to the Council's Single Multi-Agency Panel (SMAP) panel. A draft plan will be created and you will receive a copy of the plan.
