Transition pathway planning
For most young people, and especially those with an Education, Health and Care Plan, growing up and becoming an adult is a time of change and development as they move towards personal independence and set out on their life path. This process is called transition pathway planning.
It is very important that young people with SEND can access good-quality information, which will enable them to make the right decisions as they move into adulthood.
Young people may be receiving services from a number of agencies such as health, social care, voluntary agencies or education services; it is important that these organisations are involved in planning for the future so that choices and concerns can be explored properly.
The transition pathway planning process start when a young person is in Year 9. It will begin with ensuring that a young person's strengths, likes, dislikes and future aspirations are understood. Read more about the key topics covered at annual reviews.
If a young person is in contact with children's services it is really important that transition pathway planning takes into consideration the transfer to adult services if this is applicable. The ability to continue children's services beyond 18 and to join up various assessments across the Children and Families Act (2014) and the Care Act (2014) should mean a more flexible framework where transition can be a personalised evolution from the age of 14 to the age of 25.

- Further education
- Employment
- Housing
- Health and wellbeing
- Relationships
- Getting around and being part of the community
Timescales
- Year 9 - you'll be asked to think about your hopes for the future and what you want to do when you leave school, this is call your Year 9 review
- Year 10 - you'll be involved in a review of your education, career and social care plan
- Year 11 - your transition review takes place and your transition plan is agreed
- Year 12 - your annual review takes place, this will include an update on progress, any actions that need to be completed and an introduction to personal budgeting
- Year 13 - your transition and support plan, personal budget is finalised
- Age 18 - you'll transfer to adult services on your 18th birthday
