If you are a parent, carer, or professional (other than school) and you are 16 to 25 and wish to make your own request for an education, health, and care needs assessment, please complete the form via the link below.

Please give us as much information as you can, as this will assist in the decision-making process. This includes informing us of any other agencies involved with the child or young person and any reports you may have that could help support this request. To assist you, please have any reports ready to upload prior to starting the form.
