Integrated Commissioning strategies and reports
In Plymouth we are committed to working in partnership with others to enable our citizens to be able to get the right care they need, at the right time, in the right place. This means we want our children to be able to have the best possible start to life and be able to live fulfilling and independent lives as adults. We want to support people to stay well for as long as possible as they get older and be able to make choices for themselves about how they live their lives. If they need extra care and support, we want this to be really good quality, working with people and their families sensitively through difficult times.
To help us with this in 2015 we created four integrated commissioning strategies, to bring together how we want to design and commission services across health and social care.
Integrated Commissioning strategies
Integrated commissioning aims to:
- improve health and wellbeing outcomes for the local population
- reduce inequalities in health and wellbeing of the local population
- improve people's experience of care
- improve the sustainability of our health and wellbeing system
The four current strategies apply to the health and wellbeing services commissioned for Plymouth and the health services commissioned for South Hams and West Devon.
We plan to refresh the integrated commissioning strategies during 2024 and into 2025, to bring them up to date and take on board everything we have learnt since 2015, including from the COVID-19 pandemic and cost of living crisis. We need to make sure the commissioning strategies help us plan for the future and the services we will need.
The current commissioning strategies are:
Wellbeing
Children and Young People
Community
Enhanced and Specialised Care
